Consumer Health Data Privacy Policy
Effective July 19, 2026
This policy describes how Liberated Motion (“we,” “us,” “the Practice”) collects, uses, and shares consumer health data through liberatedmotion.com, as required under the Washington My Health My Data Act (RCW 19.373). This policy is separate from our Notice of Privacy Practices, which governs your clinical treatment records once you become a client. This policy covers information collected through our website, including from visitors who are not yet clients.
What Data We Collect
Through our website, we may collect:
- Contact information you provide, such as your name, email address, and phone number, when you submit our contact form or sign up for communications
- Booking information submitted through our online scheduling/booking tool, such as your name, contact details, and preferred appointment information
- Technical and usage data, such as your IP address, browser type, and pages visited, collected through site analytics
Some of this information may qualify as “consumer health data” under Washington law — for example, information suggesting you are seeking Structural Integration or bodywork services, or any health-related details you voluntarily include in a contact form message.
Why We Collect It
We collect this information to:
- Understand how visitors use our website, in order to maintain and improve it
- Respond to inquiries and communicate with prospective and current clients
- Schedule and manage appointments
Categories of Sources
We collect consumer health data directly from you, through:
- Our website contact form
- Our online booking/scheduling tool
- Voluntary email sign-up
We also collect technical/usage data automatically through website analytics tools when you browse our site.
Categories of Data Shared, and With Whom
We share limited data with the following third-party service providers, solely to operate our website and business:
- IONOS SiteAnalytics — receives technical and usage data (e.g., IP address, browsing behavior) to provide website analytics
- Booking/scheduling platform — receives the booking information you submit to schedule appointments
- Client management software — used to support scheduling and, where applicable, intake. Information submitted for the purpose of establishing your clinical treatment record is health care information governed under Washington’s Uniform Health Care Information Act (RCW 70.02) and our Notice of Privacy Practices, not this policy. Information collected through this software before that point — for example, general scheduling details for someone who has not yet become a client — remains governed by this policy
We do not sell consumer health data. We do not share consumer health data with any third parties for advertising or marketing purposes.
Your Rights
Under Washington law, you have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data, and to access that data
- Withdraw consent for our collection and sharing of your consumer health data at any time
- Request deletion of your consumer health data, which we will honor across our own records and will forward to any third party we’ve shared it with, so they can honor it as well

Changes to This Policy
If we begin collecting, using, or sharing consumer health data in ways not described in this policy, we will update this policy and obtain your affirmative consent before doing so, as required by law.
Notice of Privacy Practices
Effective July 19, 2026
Liberated Motion (“the Practice,” “we,” “us”) is a solo Structural Integration practice operated by Mihca Anderson, LMT. We are committed to treating and using your health care information responsibly. This notice describes the information we collect, how and when we use or disclose it, and your rights regarding it.
This notice applies to health care information as defined under Washington’s Uniform Health Care Information Act (RCW 70.02).
Understanding Your Health Record & Information
Each time you visit the Practice, a record of your session is created. This typically includes the date of service, health history, assessment findings, description of treatment provided, and a plan for future care. This record serves as:
- The basis for planning your care and treatment
- A legal document describing the care you received
- A means by which you or a third-party payer can verify services were provided
- A tool for the Practice to assess and improve the care we provide
Your Health Information & Rights
Although your health record is the physical property of the Practice, the information belongs to you. You have the right to:
- Obtain a paper copy of this notice upon request
- Look at or get copies of your medical information under RCW 70.02.080. Copies will be provided at no charge, or for a reasonable cost-based fee not to exceed applicable state limits; requests must be made in writing
- Amend your health record, as provided in RCW 70.02.100
- Obtain an accounting of disclosures of your health information made for purposes other than treatment, payment, and healthcare operations, and other specified exceptions
- Request a restriction on certain uses and disclosures of your information, as provided in RCW 70.02.090
- Request confidential communications — for example, being contacted at a specific phone number or address
- Revoke your authorization to use or disclose your health information, except to the extent action has already been taken in reliance on it
How We Use and Disclose Your Health Information
Treatment. Information from your intake, health history, and session notes is used to plan and guide your Structural Integration care, and to track how you’re responding to treatment over your series.
Payment. We do not bill health insurance. If you are seeking reimbursement through auto insurance Personal Injury Protection (PIP) coverage, we may disclose treatment documentation — including diagnosis, dates of service, and clinical rationale — directly to the relevant auto insurer as necessary to support your claim. Beyond PIP-related disclosures, payment for services is handled directly between you and the Practice.
Health care operations. We may use your information for internal purposes such as reviewing and improving the quality of care we provide, billing and administrative functions related to PIP claims, and recordkeeping required under Washington massage therapy regulations (WAC 246-830).
Business associates and vendors. Where we use outside vendors to support the practice (for example, client management or scheduling software), we may share limited health information with them as necessary to perform that function. We require any such vendor to appropriately safeguard your information.
Appointment reminders. We may contact you with reminders about upcoming appointments.
Other permitted or required disclosures, without your authorization, in limited circumstances:
- When required by law (e.g., court order, subpoena)
- To public health authorities as required by law
- To avert a serious and imminent threat to health or safety
- To Workers’ Compensation programs, if applicable to your claim
- To a family member, personal representative, or another person involved in your care in an emergency where you’re unable to give or refuse permission — limited to information directly necessary for your care
Any other use or disclosure not described above — including any use of your information for marketing purposes beyond appointment reminders — requires your specific written authorization, which you may revoke at any time in writing.
Our Responsibilities
The Practice is required to:
- Maintain the privacy of your health information
- Provide you with this notice of our legal duties and privacy practices
- Abide by the terms of this notice
- Notify you if we are unable to agree to a requested restriction
- Accommodate reasonable requests to communicate health information by alternative means or locations
We reserve the right to change our practices and to make a revised notice effective for all health information we maintain. If our practices change, we will make a revised notice available to you and post it at liberatedmotion.com.
